That they don&#39;t make mistakes. That they follow procedures and common sense and what they would tell you 24&#215;7 is the right thing to do &#8230; and that they do it 24&#215;7.<\/p>\n<p>But people err. People have lapses. People goof. They get tired. They get distracted. They get bored. They just &#8230; make mistakes.<\/p>\n<p>So any time we set up a policy or solution, we need to consider, realistically &#8230; <i>what mistakes could be made, and what might happen if they are?<\/i><\/p>\n<p>That way paralysis could lie.
