The page title says it all: Mystery Science Theatre 3000 Presents Jack Chick’s “Dark Dungeons”!.

(Interestingly, this is a slightly different version of “Dark Dungeons” than the version I’ve linked to before. In particular, the “preaching” pages at the end have different passages, including the reminder to burn all that nasssty Tolkien and Lewis crapola, too.)
