The only thing I'd add to the list are people who end communication links (phone calls, radio contact, video channels) by just hanging up, without any sort of good-bye or sign-off.

• Night scenes filmed during the day with a filter (OK, technical limitations of the day, but still…)

Tape recorders can produce sound when rewinding.
And most of these don't bug me at all. My suspension-of-disbelief-fu is strong. 😀
Except for the password-cracking-thing. Mild to bad annoyance, depending.
The tape recorders I have owned only made sound if I held the button partway down so the head was engaged. And it even happens with digital recordings on computers on some of these shows (which I suppose could be programmed in, but why?).
Oh, that reminds me of another one I must add. Computer displays where every action makes a sound, and where the piece of data you are looking for is highlighted.

I'll add…as the character-at-a-time password thing is a serious pet peeve of mine…that it doubly annoys me when it takes the same amount of time to "break" each character of the password.

Hmmm, to which I'll add, "Computer displays which have 96-point text on them for the key data fields or warning messages, when someone sitting at the computer really wouldn't need or get anything other than normal text size."
